MCI Vancouver Office has been assigned as the Official Housing Bureau for the e-Health 2012 Conference – “Innovating Health e-Care”.
We would like to request your assistance and support of e-Health 2012 by booking your accommodations exclusively at the conference hotels. The official conference hotels were chosen for the numerous benefits they offered the conference as well as the attendees. Your loyalty and co-operation are greatly appreciated.
Hotel Reservations Official Conference hotels will not accept reservations directly. Reservations must be made on-line through MCI Vancouver Office’s secure website for instant confirmation, this site is accessible once registration is complete or by following the link via the hotel information page located on the conference website.
All fees are in Canadian dollars (CAD). Our secure server will process MasterCard, Visa, and American Express. Charges on your credit card statement will appear as ADVANCE GROUP.
Please note, there is an additional Offline Payment Processing Fee of $50.00 CAD (+ 12% HST), to be added to Cheque & Wire Transfer payments.
Cheques are to be made payable to: e-Health 2012 c/o Advance Group and must accompany your completed form and received no later than May 25, 2012.

• Participants may make changes online at any time by accessing their e-Health 2012 Registration Profile. • Any changes requiring assistance from e-Health 2012 Secretariat are subject to a $60.00 CAD (+ 12 %HST) administrative fee. • An alternate attendee name may be substituted if you are unable to attend the Conference. You can do this without incurring any fees by logging into your e-Health 2012 Registration Profile and substituting the relevant information. Substitutions requiring assistance from the Secretariat are subject to a $60.00 CAD (+ 12% HST) administrative fee.
CANCELLATION NOTIFICATION & REFUNDS
• Notification by 5:00 PM PT April 27, 2012 - A refund will be granted less a $60.00 CAD (+ 12% HST) administration fee and a $100.00 CAD (+ 12% HST) cancellation fee per registration. • Notifications received after 5:00 PM PT April 27, 2012 are 100% non-refundable. CPHIMS - CA Exams Registrations Fees are non-refundable.
RE-VERIFICATION OR REPROCESSING OF A CREDIT CARD
• Credit card payments that require reprocessing for any reason will incur a $75.00 CAD (+ 12% HST) administrative fee.
By attending the e-Health 2012 Conference, the participant acknowledges that he/she may be photographed and subsequently may appear in photographs that may be used by the host organizations of the e-Health Conference for promotional purposes of future events.
